Optum credentials massage therapists individually, so each provider completes a few items before we submit their packet. Do them in order and you'll be ready fast.
The short version
Get your NPI, keep your Florida license & malpractice insurance active, and complete your CAQH profile (and authorize Optum). Then we handle the W-9, reassignment of benefits, and submitting your packet.
The steps
Confirm your Florida license
Make sure your FL Massage Therapist (MA) license is active; renew if it's within ~90 days of expiring. Verify at flhealthsource.gov.
Get your individual NPI (Type 1)
Free, and it takes about 10 business days. Create an Identity & Access (I&A) account, then apply at nppes.cms.hhs.gov. Use taxonomy 225700000X (Massage Therapist). Already have an NPI? Just send it to us.
Professional liability (malpractice) insurance
Keep active coverage and have a certificate / face sheet ready to upload to CAQH. Many therapists get this through ABMP or AMTA.
Complete your CAQH profile (and authorize Optum)
Free, and about 2–4 hours the first time. Set up or update your profile at proview.caqh.org, "attest" (the final confirmation), and authorize Optum to access it. This is the single biggest thing that keeps your file moving.
We handle the rest
Once the above is done, Issac handles your W-9, application packet, reassignment of benefits, and submitting the credentialing packet to Optum.
